What are your fees?
Our regular fees are $130 per 45 minute individual session, $150 per 55 minute individual session, and $160 per 55 minute conjoint/couples session. The initial evaluation period, which can last from one to three 60 minutes sessions, is billed at $210.00 per session.
We charge our agreed upon hourly fee for other professional services you may need, though we will break down the hourly cost if we work for periods of less than one hour. Other services include: report writing, telephone conversations other than for scheduling appointments, attendance at meetings with other professionals you have authorized, preparation of records or treatment summaries, and the time spent performing any other service you may request of me. Clients will be given 30-day verbal notice prior to any change in fee structure.
If you do not have insurance coverage or do not want to use your benefits, and you cannot afford our regular fees, we offer a few sliding scale fee spots based on your income.
We accept cash or personal checks and payment is due at the time of service (including your copay/deductible/coinsurance payments). All checks should be made payable to "Chicago Center for Psychotherapy, LLC," or to "CCP," and should be mailed to Chicago Center for Psychotherapy, 137 N. Oak Park Avenue, Suite 327, Oak Park, IL 60301.
If you would like to use your credit card to pay for your session, just click on the "Pay Now" button below. This will take you to the secure PayPal site and you can enter the amount of your payment and a description of the payment (e.g., copay for 12.24.16). A 2.9% convenience fee will be automatically added to your payment (this fee is collected by PayPal, not CCP). You do not need to have a PayPal account. Please make your payment prior to your appointment, as payment is due at time of session.
PayPal can be used to pay for “self-pay” sessions or for paying co-payments/co-insurance and deductibles. All major credit cards are accepted by PayPal.
How long are therapy sessions?
Sessions are either 45 minutes or 50-60 minutes in length, except for the initial evaluation period, which can last from one to three 60 minutes sessions.
How often are therapy sessions scheduled?
We usually see clients once per week, but offer twice weekly sessions for clients in crisis.
How can I schedule my first appointment?
Scheduling an appointment can be done by calling the office at 708.848.0491, or by emailing CCP's Practice Manager, Constance Bauer at email@example.com. If you do not reach her directly, please leave a confidential message on the voice mail. Please be aware that confidentiality cannot be assured for email communication. Please leave your name, phone number, and the best times to reach you and Constance will return your call or email as soon as possible.
What paperwork will I need to complete?
Before your first appointment, you will need to complete and sign the CCP New Client Agreement, and read and sign the CCP HIPAA (Privacy) Consent Form. If you plan to use your BCBS insurance, you will also need to complete and sign the CCP BCBS Benefit Information Form. If you will be using other insurance, or will be paying out of pocket, you will need to complete the CCP Self-Pay Demographics Form. For couples, each individual partner must complete their own CCP New Client Agreement and HIPAA (Privacy) form. The CCP BCBS Benefits Form or Self-Pay Demographics Form should be completed by the partner whose insurance you are planning to use.
